IWS Family Health, a community health center providing preventive health care services to children and families, seeks a Family Medicine Nurse Practitioner to help grow our practice at our primary location in Logan Square.
Reporting to our Family Medicine Director, this role provides all aspects of well and sick care to the patients of IWS. The Nurse Practitioner will support evidence-based practice, contribute to performance measurement and clinical audits, engage in continuous quality improvement activities, and help ensure the delivery of safe, effective, and patient-centered care in alignment with organizational standards and regulatory requirements.

Reporting to our Family Medicine Director, this role provides all aspects of well and sick care to the patients of IWS. The Nurse Practitioner will support evidence-based practice, contribute to performance measurement and clinical audits, engage in continuous quality improvement activities, and help ensure the delivery of safe, effective, and patient-centered care in alignment with organizational standards and regulatory requirements.
What you will do:
- Deliver high quality, patient-centered care in an FQHC look-a-like setting via in-person visits and telehealth.
- Conduct well visits, immunizations, and acute care visits for all ages from newborn to geriatrics.
- Diagnose and manage illnesses, chronic conditions, and behavioral concerns.
- Participate in 24-hour call rotation with other providers.
- Review laboratory results, referral results, and emergency room discharge, and follow-up appropriately.
- Collaborate with nurses, behavioral health and dental providers, clinical support staff, and specialists to ensure comprehensive and coordinated care.
- Assist in education of patients, residents, medical students, and other staff as appropriate.
- Apply a continuous quality improvement approach to care and participate in quality initiatives.
- Maintain accurate, timely, and compliant electronic health records (EHR).
- Participate in community outreach events as needed.
- Adhere to FQHC regulatory requirements.
- Collaborate with others to ensure compliance with IWS funding and grant requirements.
What you will bring:
At a minimum
- MS in Nursing or other related field
- Current APN License in the state of Illinois
- Three years of clinical experience
- Bi-lingual language skills would be a great benefit to our patients. Spanish and Polish are the most common non-English languages.
