IWS Family Health, a community health center providing preventive health care services to children and families, seeks a Behavioral Health Integrated Counseling Therapist to work as a key member of a multidisciplinary team, providing direct clinical services and support to patients within a healthcare setting.

Reporting to our Director of Behavioral Health Integrated Counseling, this role is centered on offering therapeutic interventions, crisis response, and assisting with the development and implementation of treatment plans, with an emphasis on using evidence-based practices to support patient wellbeing.

What you will do:
 
  • Intake and Treatment Planning: Manage patient referrals within the BHIC program, conducting intakes and developing individualized treatment plans using evidence-based models and approaches.
  • Therapeutic Responsibilities: Conduct crisis interventions, evaluations, and short-term therapy services to provide immediate support and stabilize patients in need.
  • Crisis Response: Provide immediate support during crisis situations, including coordinating with external agencies such as the Department of Children and Family Services (DCFS) and Screening, Assessment, and Support Services (SASS).
  • Documentation: Accurately document services provided to patients in the Electronic Medical Record (EMR) system, ensuring compliance with all regulatory and organizational standards.
  • Multidisciplinary Team Collaboration: Work as an integral part of a team to assess and support the needs of patients, coordinating care across disciplines.
    • Service Tracking and Reporting: Maintain accurate records of services provided and programs developed, ensuring proper tracking and reporting for program outcomes.
    • Screening and Referral Support: Review and interpret the results of various screening tools (e.g., Edinburgh, PHQ-9, GAD-7) and support patients by recommending treatment options, coordinating referrals, and facilitating warm handoffs to long-term therapy, when appropriate.
  • Training and Presentations: Participate in the development and delivery of sensitive topic presentations to clinic staff to enhance understanding and support of patient care needs.
  • Team Meetings: Participate in regular departmental and clinic staff meetings to discuss patient care, ongoing initiatives, and team dynamics.
  • Additional Tasks: Perform other duties as assigned by the supervisor that are in line with the role and consistent with agency policies and procedures.

What you will bring:

At a minimum
  • Licensed Social Worker (LSW)
  • Master’s Degree in Social Work
  • Bilingual: Must speak and write English/Spanish or English/Polish
  • Proficient with Microsoft Office software
  • Excellent communication and interpersonal skills

We are hoping for
  • Previous experience in health care setting, including work as a mental health professional, case manager or health educator
  • Experience working with an Electronic Medical Record (EMR)
  • Clinical background with familiarity of DSM-V and screening/assessments

What we are offering:

IWS Family Health provides a competitive salary and benefits plan, including Paid Time Off (PTO) starting at 23 days each year, 7 paid holidays and 2 floating holidays.  We offer health, dental, life, disability and vision insurance options, as well as a 403(b) program. The annual salary for this role is $50,000 - $60,000.

In addition, we are happy to provide supervision for clinical hours to pursue your LCSW.
